Aloha,
It’s that time of year for the Annual Medfest at Kaiser Permanente Mapunapuna. Over the past nine years, the Kaiser Permanente team of physicians and health professionals have donated their time, expertise, and facilities to offer the best medical screenings and support to SOHI athletes. We hope that athletes will take advantage of this wonderful opportunity. Please see the flyer attached for details.
What: Medfest – Oahu
When: February 11, 2025, Tuesday
Where: Kaiser Permanente – Mapunapuna Clinic
Time: 8:30 am to 12:00 pm
The Medfest screening is open to all School Delegations and Adult Athletes with expiring medical documents.
- School delegations – Contact Emerald-Leigh Chun for bus transportation at unify@sohawaii.org.
- Adult Athletes please arrange schedule & transportation to attend available dates and times.
Registering an appointment with Kaiser Permanente’s team of amazing physicians is required.

The following documents need to be completed before the Medfest screening.
New Athletes:
- Athlete Registration Form
- Athlete Release Form – Athlete Release Form Must be signed by athlete/parent or guardian.
- Covid Communicable Disease Release Form
- Athlete Medical Form – please note EVERY question from pages 1-3 MUST be filled out
Returning Athletes:
- Athlete Registration Form
- Athlete Release Form (if unsure if previously completed)
- Covid Communicable Disease Release Form
- Athlete Medical Form – please note EVERY question from pages 1-3 MUST be filled out
